Health Certificate Request

Health Certificates must be issued within 10 days of travel and are valid for 30 days. For cabin travel on Alaska Airlines or travel within Alaska on Alaska Airlines, a certificate may not be required. We do not offer Hawaii travel certificates. If you have a current rabies vaccination, please upload the certificate below or email it to [email protected].
